If you are a resident of the State of New York or Rhode Island, you are required to sign an affidavit and send it with your Dutch samples when you send them to the lab.
New York and Rhode Island do not allow their residents to do certain at-home lab test kits.
The affidavit verifies that you were outside of NY or RI when you collected your urine sample. You must print out this affidavit, sign it, and send it in the pre-paid envelope back to the Dutch lab with your dried urine samples. Dutch will then be able to process your labs.
